Post Internal Days 0 Category Family Foster Care Description CATHOLIC GUARDIAN SERVICESJOB DESCRIPTIONPOSITION TITLE Recertification WorkerDEPARTMENT Family Foster CareQUALIFICATIONS BA with minimum two (2) years experience in Child WelfareBASIC FUNCTION OF POSITIONUnder supervision, study and evaluate the foster family functioning for continued approval or licensing as a foster parent with Catholic Guardian Services. Such evaluation is for the purposes of ensuring continued adherence to State, City and Agency policy and the overall safety and well being of children placed within the foster home setting.BASIC DUTIES AND RESPONSIBILITIES
  • Runs and reviews Resource Directory Inquiry Tool (RDIT) and Data Warehouse reports within required time frames.
  • Responsible for sending recertification packages to foster parents within the 120 days prior to recertification.
  • Responsible for reviewing foster parent record and Connections system to ensure documentation including but not limited to, medical reports, child abuse clearances, fingerprints, safety assessments, training, financial information and other required documents are up to date.
  • Responsible to ensure follow up with aforementioned documentation if assessed to be outstanding.
  • Responsible to ensure that household members age 18 and over are fingerprinted in accordance with State regulations including foster children or household members newly turning of age.
  • Responsible to ensure that any new household members to the foster home are assessed and submit required documentation.
  • Responsible to conduct home visits to ensure the safety of the physical plant and to conduct a face to face interview with the foster parent(s) and household members. Visits may be at least once annually or more if required i.e. the home requires additional inspection or if a foster parent moves to a new location
  • Responsible to submit a completed written narrative to the supervisor within 30 days of home recertification to include the evaluation of the physical plant and foster family functioning.
  • Responsible to correspond as necessary with case planning staff or other collateral individuals to assess foster family functioning.
  • Responsible to correspond as necessary with case planning staff regarding any issues of concern that may impact the re-licensing or re-approval of the foster home.
  • Responsible to report any issue of safety to the Director of Homefinding Services or other appropriate senior staff person. Report may also include a report to the State Central Register when there is a reasonable cause to suspect child abuse or neglect.
  • Responsible to update the Connections and MyEvolv data systems regarding foster family information.
  • Attends weekly supervisory conferences with Director of Homefinding Services or more often if necessary and required.
  • Responsible to attend meetings with foster parents or household members as necessary should any issues of concern need to be addressed.
  • All other duties as assigned.
