Location: Fully Remote in New York City, NY (20% travel for quarterly leadership summits)

What You'll Tackle
- Lead cross-functional projects that streamline the patient journey from hospital discharge through home-based recovery, driving measurable gains in speed, quality, and cost.
- Build and refine scalable playbooks, KPIs, and compliance processes to support multi-state growth while maintaining best-in-class clinical standards.
- Act as the right-hand partner to the VP of Client Success & Strategy, translating high-level vision into executable 30-60-90-day roadmaps.
- Coach and develop regional managers, fostering a culture of accountability, transparency, and continuous improvement.
- Evaluate and deploy technology solutions (EHR, scheduling, BI dashboards) that enable data-driven operational decisions.
- Own executive-level reporting - distill complex data into concise updates and recommendations for the C-suite and board.
- Serve as the organization's senior problem-solver, making informed operational calls in a fast-moving environment.
- Champion patient-centric values in every process you touch, from SOP creation to vendor selection.
What You Bring
- 8+ years of progressive multi-site healthcare operations leadership, ideally in post-acute, home health, or allied services.
- Demonstrated success scaling processes across state lines while keeping compliance tight.
- Entrepreneurial operator comfortable with ambiguity, rapid growth, and rolling up sleeves.
- Executive presence - you influence without formal authority and can present to senior leadership with ease.
- Proven ability to develop teams and create high-performing, metrics-driven cultures.
- Bachelor's degree required; advanced clinical or business credential a plus.
