Become the Case Manager You Aspire to Be

In this role, you will:

  • Collaborate with an interdisciplinary team to guide treatment plans tailored to patient needs.
  • Coordinate with the interdisciplinary team to establish a discharge plan and back-up plans.
  • Play an active role in planning and executing the patient discharge experience.
  • Monitor various aspects of patient care, including quality, timeliness, service appropriateness, and payer expectations.
  • Facilitate weekly team conferences and coordinate treatment plan modifications.
  • Complete case management documentation as required.
  • Stay updated on regulations, company policies, and department operations.
  • Analyze case management reports and implement necessary actions.
  • Understand commercial contracts, exclusions, and payor requirements.
  • Attend Acute Care Transfer meetings to identify trends and reduce ACTs collaboratively.
  • Engage with patients and their families within 24 hours of admission.
  • Assess patient goals and complete case management documentation within 48 hours of admission.
  • Educate patients and families about rehabilitation and the Case Manager role, establishing a communication plan.
  • Schedule and facilitate family conferences as required.
  • Assist patients in securing resources promptly to prevent discharge delays.
  • Ensure compliance with regulations regarding orthotics and prosthetics.
  • Make timely referrals to post-discharge providers and document appropriately.
  • Verify the accuracy of discharge and payer-related information.
  • Participate in the utilization review process, including data collection, trend review, and corrective actions.
  • Contribute to case management on-call responsibilities as needed.

Qualifications

  • License or Certification:
    • Must independently complete assessments within the scope of practice (e.g., RN, SW, OT, PT, ST, and Rehabilitation Counseling).
    • If licensure is required for the discipline, an active license must be maintained.
    • CCM® or ACM certification must be obtained within two years of entry into the position.
  • Minimum Qualifications:
    • For Nursing: an Associate Degree in Nursing is required; RN licensure is mandatory, with BSN preferred. A diploma can be accepted in states where that is the minimum requirement.
    • For other eligible licensed or certified health care professionals: a bachelor's degree is required, with a graduate degree preferred.
    • Two years of rehabilitation experience is preferred.
