This position will entail working with children, youth, and families with complex needs, who would benefit from intensive, highly individualized planning and care coordination with the goal of moving the families towards independence and success. You will identify needs, develop a support team, develop individualized and strengths-based plans, and coordinate services and supports. The Wraparound Facilitator will work closely with all disciples on the team, to include the educational system if needed, with the purpose of optimizing collaboration and integration of care. The team will promote the strengths, voice, and culture of the family to build confidence and experience success at home, in school, and out in the community. This role is primarily remote, with some flexibility in creating your weekly schedule. Why You Will Love Working for CFCI:

Wraparound Facilitator Responsibilities:
  • Facilitate the development of an individualized plan of care for each child/family that includes clear needs statements, strength-based strategies, and specific outcomes
  • Coordinate and facilitate ongoing child and family team meetings
  • Meets face-to-face with clients in their own home if appropriate / video meetings encouraged at this time
  • Keep the family and team members informed on progress toward achieving specified goals
  • Continuously reassess the needs of the child and family with the team and modify the treatment plan as needed
  • Act as liaison when new services or resources are needed
  • Maintain an empathetic, yet professional relationship with children and families that strives to keep families engaged in all services
  • Caseload will be a maximum of 10 children/families
  • Evaluate client through conversations, interviews, and observations to determine correct testing or examination
  • Refer patients to resources like psychiatrists, treatment facilities, or community services
  • Maintain good working relationships with support networks, government resources, and community resources
  • Complete the Wraparound Facilitator training and credentialing process
  • Obtain and maintain CANS (Child and Adolescent Needs and Strengths) certification, including completing all required training and assessments
Wraparound Facilitator Requirements and Qualifications:
  • Excellent interpersonal skills with patients, colleagues, and community resources
  • Compassionate and caring of all clients
  • Excellent time management skills; organized and able to prioritize
  • Comfortable in a fast-paced environment with multiple clients
  • Able to organize and manage large amounts of files, schedules, dates, and information
  • Calm and cool-headed with a positive attitude
  • Self-directed and self-motivated
  • Empathetic and supportive with mentorship and leadership skills
  • Proficient computer skills, including Microsoft Office and Google docs
  • Dedicated to the success of children and family teams
Requirements:
  • Master's degree in Counseling or related subject
  • Proven leadership abilities
  • Patience
  • Ability to interact well with children and their families
  • Excellent writing and communication skills
  • Strong organizational skills
  • Knowledge of child development
Pay Range
  • $25-$50/hour, based on licensure and years of experience
  • Initial 90 day salary may be offered while caseload is being established
