Day in the Life of a Care Coordinator

Crossroads is a family of professionals dedicated to providing the most accessible, highest quality, evidence-based medication assisted treatment (MAT) options to combat the growing opioid epidemic and helping people with opioid use disorder start their path to recovery. This comprehensive approach to treatment, the gold standard in care for opioid use disorder, has been shown to prevent more deaths from overdose and lead to long-term recovery. Our clinics are all outpatient and office-based, with clinics in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia. Responsibilities
- Meet face-to-face with all patients to assist in the facilitation of recovery services and ensure that all recovery activities are properly documented in the patient's record.
- Utilize collateral contacts and other local referral agencies to provide services not offered by the program.
- Conduct monthly Care Coordination meetings with patients, and complete corresponding documentation, to assure that patients are engaged with the services they need.
- Working in collaboration with the buprenorphine-waivered practitioner who is prescribing buprenorphine or naltrexone products to individuals with moderate to severe opioid use disorder.
- Perform other duties as assigned by the Operations Director of OBOT Services.
Education and Experience Requirements
- A bachelor's degree in social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, or human services counseling and at least either one year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness.

- Thursday; 9:00am-5:00pm
- Friday; 9:00am-3:00pm
