As a Case Management Manager, you'll play a vital role in overseeing our Care Coordination department. Key Responsibilities:

  • Lead the daily operations of the Care Coordination department, ensuring efficient workflows among RN Case Managers, Social Workers, and admin staff.
  • Set and implement departmental goals and standards aligned with our strategic vision.
  • Assess and enhance the quality of patient care, facilitating communication across teams for optimal patient outcomes.
  • Provide mentorship and ongoing training for staff to foster professional growth and compliance with policies.
  • Monitor departmental budgets and ensure adherence to regulatory requirements.
  • Manage staff relations, including performance evaluations, conflict resolution, and recruitment.
  • Utilize evidence-based practices and scientific research to inform nursing practices and patient care strategies.
  • Engage in continuous safety monitoring and address any gaps in processes that could hinder efficiency.

Qualifications:

  • Bachelor's degree in Nursing (BSN) or a Master's in Social Work (MSW) is required.
  • A minimum of 5 years of experience, including 2 years in a leadership role or progressive management experience.
  • Required: State licensure as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) appropriate for the state of work.

Physical Requirements:

  • Ability to lift and carry objects weighing up to 50 lbs, as well as the capability to frequently move throughout a healthcare setting.
  • Regular use of vision and hearing for effective communication and monitoring.

Work Shift: Day Shift

Scheduled Weekly Hours: 40
