Your Role as a Case Manager

  • Work collaboratively with an interdisciplinary team to tailor treatment plans according to patient needs.
  • Coordinate the tentative discharge plan and prepare contingency strategies as needed.
  • Be an advocate for patients throughout their discharge experience.
  • Monitor patient care quality, timeliness, and expectations.
  • Lead weekly team conferences to adjust treatment plans accordingly.
  • Complete necessary case management documentation with attention to detail.
  • Stay informed about regulations, company policies, and operational standards.
  • Evaluate case management reports and determine necessary actions.
  • Understand commercial contract requirements and plan for recertification.
  • Engage in Acute Care Transfer (ACT) meetings to identify improvement trends.
  • Meet with patients and families, ensuring effective communication and support.
  • Provide timely resource planning to avoid discharge delays.
  • Oversee compliance with regulations regarding orthotics and prosthetics.
  • Facilitate timely referrals to post-discharge providers.
  • Ensure accurate documentation of discharge and payor-related information.
  • Participate in utilization review processes to enhance performance.
  • Join the on-call schedule as needed.

Qualifications

License or Certification:

  • Must be eligible to independently conduct assessments within respective professional disciplines (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
  • If licensure is necessary, an active license must be held.
  • Must meet requirements for CCM® or ACM certification upon entry into this position or within two years.
  • CCM® or ACM certification required within two years of starting the role.

Minimum Qualifications:

  • For Nursing, an Associate Degree in Nursing with RN licensure is required; BSN preferred.
  • For other licensed healthcare professionals, a minimum of a bachelor's degree is required; a graduate degree is preferred.
  • Preferably 2 years of rehabilitation experience.
