Job Title: Registered Nurse – Home Health (Contract)Location: Henry Ford Allegiance Health, Jackson, MISchedule: 40 hrs/week | 8-hr day shifts | 5 days/week | 8a - 4:30p

13-week contractPay Range: mileage reimbursement ($0.67/mile)

Overview:The Home Health RN provides direct patient care in a home setting, ensuring safe, quality, and integrated care delivery. This role requires strong clinical judgment, patient education skills, and the ability to collaborate with interdisciplinary providers to support comprehensive home health services.

Responsibilities:

• Provide skilled nursing care in patients' homes.
• Assess patient needs and coordinate referrals for allied health services (PT, OT, nutrition, speech, social work).
• Document assessments and interventions accurately using OASIS and Epic.
• Administer IV therapy and perform other specialized home health procedures.
• Educate patients and families to promote self-care and independence.
• Travel to patient homes (mileage reimbursed).

Requirements:

• Active Michigan RN license
• Current BLS (AHA)
• Minimum 2 years of home health experience
• IV and OASIS documentation experience
• Epic EHR experience
• Valid Michigan driver's license, reliable transportation, and satisfactory driving record

Preferred:

• Prior acute care experience
