Grade/Salary Range: A13 ($19.12 - $23.23/hourly) - Non-exempt * salary commensurate with experience Position Summary: Under the direction of the Maternal and Child Health Navigation Supervisor, the Neighborhood Navigator is responsible for identifying and engaging at-risk women and connecting them to comprehensive clinical care and other needed services. Navigators will facilitate access to services by acting as a liaison between health and social services and the community by developing and maintaining a portfolio of resources. Responsibilities include but not limited to: Performing care coordination and documentation through outreach and home visits to perform health assessments of clients and educate them on various health topics; Responsible for establishing trusting relationships with women and their families while providing general support and encouragement; Ensuring attendance at trainings and/or quality assurance activities. Directing, monitoring, and connecting pregnant women and families with each other and services available within the agency and community; Recruiting to generate referrals to enroll African American pregnant women into the program Conducting intake interviews with women including use of the Ohio Department of Children & Youth's screening tool. Developing and maintaining a portfolio of local referral resources. Using and utilizing evidence-based interventions, clinical, and social services. Serving as a role model, mentor, and advocate for women and their families. Providing follow-up to women via phone calls and other means as necessary to ensure needs are met. Staying up to date and knowledgeable about community resources appropriate to address the needs of women/families. Documenting women/family referrals and addressing risk status via the required Department of Children & Youth (DCY) identified/approved platform. Exhibiting excellent working relationships with women, visitors, and staff. Coordinating with the Maternal and Child Health Navigation Supervisor to review documented barriers and access challenges. Minimum Qualifications: High School diploma or equivalent with course work in office practices One (1) - Two (2) years of community-based work in a community health or social service environment Successfully completed a Certified Community Health Worker Training Program; and maintaining certification related to prenatal outreach (or ability to complete within one year of hire) Experience working in a multi-cultural setting Ability to initiate and maintain positive working relationships with staff and other organizations Ability and willingness to provide emotional support, encouragement, and motivation to women Understands the community served – community connectedness Exhibits interpersonal skills that foster trust, sensitivity, empathy, flexibility, accessibility, patience, resourcefulness, enthusiasm, and the ability to be non-judgmental Understands and acknowledges that Public Health - Dayton & Montgomery County uses evidence-based and evidence-informed public health practices based on scientific research including health promotion theory and practice, environmental health measures, and communicable disease control measures including vaccination, isolation, and quarantine practices, and medicine for treatment and prevention of disease, among others. Understands and acknowledges the use of tobacco or nicotine products in any form including replacement therapy products and prescription medications used to treat smoking addiction for anyone hired after January 1, 2014, is prohibited. Required to play an active role during a public health emergency, crisis, outbreak, incident, or event in which the Health Commissioner deems additional personnel resources are warranted. This may include changes in responsibilities and working locations/hours Completion of Incident Command System 100 and National Incident Management System 700 courses within 90 days of hire. Valid State of Ohio driver's license and insurance on any personal vehicle that will be used for work.
