A busy New York hospital is seeking a Case Worker to support operations within their Social Work team. Under supervision, this individual will provide social services to clients/patients. Groups served include recipients of public assistance, food stamps and/or medical assistance; and, adults receiving or needing institutional care or protective services. This role will identify, develop, and implement social service plans for families and individuals. In addition, the individual in this role will determine the eligibility for these services and may perform counselling and investigatory activity related to the work.
