A social services organization in New York is currently seeking two APS Case Managers to join their team. These positions involve 100% fieldwork across the Bronx and Manhattan boroughs. Ideal candidates will have experience in case management and a passion for community support.About the Opportunity:100% FieldworkSchedule: Monday-FridayHours: 9:00 AM-5:00 PM, 35-hour work weekSetting: Temp-to-Perm (6 months temp, then conversion)Responsibilities:Providing direct client services, including intake and assessment.Managing a caseload and documenting progress in APSNET and ETO.Monitoring risk and adapting service plans as needed.Referring clients to appropriate external services.Conducting home visits and telephonic communication weekly.Qualifications:Bachelor's degree in Social Work or related field.Minimum of 1 year of case management experience; 4 years preferred.Enrollment in MSW program is a plus.English proficiency; bilingual in English/Spanish is a plus.Strong communication, time management, and organizational skills.Proficiency in Microsoft Office and standard business technology.Physical ability to conduct fieldwork and lift/move up to 10 lbs.by Jobble
