Using a patient-centered approach, Health Home Care Coordinators assist individuals enrolled in the program manage their conditions to achieve optimal health outcomes by developing individualized care plans designed to link individuals to necessary services; such as, housing, health care services, mental health and substance use providers, social services (food, entitlement benefits, transportation), community programs, and more. The ideal candidate for this position possesses excellent written and verbal communication skills, knowledge of community resources, and strong time management/organizational skills. The person that assumes this position demonstrates computer proficiency, good judgement, establishes healthy boundaries, has the ability to work independently, and adheres to strict confidentiality guidelines.
