Lead Care Manager – Enhanced Care Management (ECM) | Santa Clara County
A leading healthcare organization is seeking a Lead Care Manager to support its Enhanced Care Management (ECM) program in Santa Clara County. This role focuses on whole-person care by addressing not only medical needs, but also social determinants of health such as housing, food insecurity, and behavioral health.
The Lead Care Manager will play a critical role in guiding members through every step of their care journey, coordinating services, and ensuring access to vital community resources. Key Responsibilities
- Conduct frequent in-person visits in members' homes, shelters, and community settings to build trust, complete assessments, and address needs in real time.
- Develop and implement individualized care plans, coordinating across medical, behavioral health, housing, and social service systems.
- Arrange and monitor services such as medical appointments, prescription fulfillment, transportation, home health visits, meal delivery, housing assistance, and job training.
- Advocate for members by navigating insurance authorizations, appeals, and access to timely services.
- Serve as the central point of communication between members, families, healthcare providers, and community organizations.
- Maintain accurate documentation of assessments, care plans, progress, and outcomes.
- Research and connect members with community resources that support stability, independence, and long-term health.
- Ensure compliance with Medi-Cal, CalAIM, and ECM regulations and quality standards.
- Provide leadership and support to team members, contributing to continuous improvement of care delivery practices.
Qualifications
- 3–5 years of experience in case management, social services, or healthcare.
- Strong understanding of Medi-Cal, CalAIM, and Enhanced Care Management programs.
- Knowledge of healthcare systems and local community resources in Santa Clara County.
- Skilled in care planning, service coordination, motivational interviewing, and client advocacy.
- Strong organizational skills and ability to manage multiple priorities.
- Technical proficiency with case management systems and documentation.
- Residency in Santa Clara County required.
Schedule
- Full-time, Monday–Friday
- 8:30 AM – 5:00 PM PST
