**SALARY- $55K - $70K per year (depending on experience)**
Job Description:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
- Must have BSW or MSW
- Must have experience as a Staff Social Worker
- Must have long-term care/nursing home experience
- Must have excellent outreach, assessment, written and verbal communication skills.
- Must be able to work effectively as a team.
