NOW HIRING: Bilingual Field Based Case ManagerPay: $29/hour + mileage reimbursementSchedule: Monday–Friday, 8 AM–5 PMLocation: Must live in Florida – specifically near West Palm Beach or Lake Worth (this is a field-based role with home visits)Job Overview:We're looking for a compassionate and organized Bilingual Care Manager to help people who need long-term care get the services and support they need. In this role, you'll visit members in their homes, help them create care plans, and work with doctors and service providers to make sure they receive proper care. After your visits, you'll complete documentation from home.What You'll Do:Visit members in their homes to assess needs and create care plansHelp members understand their healthcare benefits and available servicesCoordinate care with doctors, specialists, and local resourcesSupport members with services like housing, employment, or independent livingUpdate care plans and document visits within 24 hoursCommunicate clearly with your team and healthcare partnersUse electronic systems to track and manage member informationBe ready to handle tech issues or visit an office if neededWho We're Looking For:Must live in Florida, preferably near West Palm Beach/Lake WorthBilingual in English & Spanish strongly preferredAt least 2 years of field-based case management experienceExperience with Medicaid/Medicare, home health, or long-term careAble to manage a large caseload in a fast-paced environmentComfortable with electronic medical records (TruCare experience is a plus)A Bachelor's degree is required (or similar experience)Friendly, organized, problem-solver, and passionate about helping othersApply TODAY or Contact:Alliston Brown | 586-###-#### (call/text)
