Job Description :

CHWs engage one-on-one with clients and their families and social support networks to support the client's engagement with their health goals and priorities through community outreach, home visits, navigation, ongoing communication and advocacy. The CHW works primarily in community-based settings as an extension of the hospital or clinic, but can also work in the clinical setting to improve access to care on-site. Essential Functions: * Deliver patient outreach, education, advocacy and support, primarily in a community setting, related to the patient's care goals and to assist with closing care gaps. *Maintain a client caseload, as determined by the Population Health department priorities. * Assist clients in their homes, community, or clinic setting to address their health-related social needs and improve engagement with their care management plans. * Help clients identify and prioritize health and socio-economic issues that affect their overall health and develop health/social management plans and goals * Foster partnerships with community-based organizations, health systems, and community stakeholders to establish and support referral pipelines for clients * Create an action plan reinforcement session to help participants achieve their health goals. * Help connect residents to social services and support (Health Insurance, SNAP, legal aide, etc.), including by facilitating timely appointments and accompanying them to appointments * Accompany clients to medical and social service visits as needed * Prepare case-related reports including, but not limited to, outcomes, successes, and challenges Conduct basic health assessments, screenings, and plans * Conduct outreach and distribute marketing materials, such as fact sheets, and outreach flyers to promote the program in the targeted areas served * Utilize community engagement and recruitment strategies * Participate in health fairs and other events in the community to promote services and improve the visibility of the program * Educate clients on community organizing and mobilization basics * Promote leadership development and wellness capacity building for community members * Facilitate and/or support educational workshops and group wellness activities * Consistently track all required outcomes, using the programs data system, Excel, and other software as required. * Active participation in case conferences and meetings * Attend required training and maintain current knowledge of information relevant to the program * Assist with other tasks and activities as needed * Engagement Methods and Strategies * Individual and Community Assessment * Culturally and Linguistically Appropriate Responsiveness * Promote Health and Well-Being * Care Coordination and System Navigation Educational Requirements: GED, High School Diploma, or equivalent Schedule: Monday-Friday, 9AM-5PM with options for some remote access Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
