Case Manager
: Job Details :

Northbridge is now hiring a full-time Case Manager to serve individuals with intellectual and developmental disabilities. 

The ideal candidate will be 21 years of age or older, have a vehicle with valid registration and insurance. Hold a valid Alaska State Driver's License and have at least two years with a good driving record. Care Coordinator training and/or experience. A minimum of two years in the Human Services field. Knowledge of Medicaid waivers. The ability to pass a State and Federal Background Check.

Job Duties:

--Review case notes for quality and timeliness

--Communicating effectively with individuals' teams

--Participating in individual Support Plan meetings and ensuring quality documentation is provided to pertinent team members

--Monitoring and updating Behavior Support Plans, Assisted Living Plans, Plan of Care, etc. as needed or required

--Interviewing applicants

--Ensuring compliance with state and federal regulations regarding documentation, individual files, trainings, etc.

--Maintain an organized work area

--Conducting home visits with individuals

--Understanding of state regulations.

--Other duties as assigned
