Designated Coordinator – Full Time

BrightPath LLC is seeking a compassionate and organized Designated Coordinator to support individuals with disabilities and older adults in community-based residential programs. This role plays a key part in ensuring services are delivered with dignity, respect, and full compliance with Minnesota's 245D licensing standards.

About the Role

As a Designated Coordinator, you'll oversee day-to-day service delivery for a caseload of individuals receiving supports. You'll provide guidance to frontline staff, ensure compliance with support plans and licensing standards, and act as a liaison between program teams, families, and external professionals. The ideal candidate is organized, supportive, and passionate about person-centered care.

To qualify for this role within our Residential Services programs, candidates must have prior experience as a Designated Coordinator in a setting that supports individuals with high behavioral support needs.

Key Responsibilities
  • Provide oversight and coordination of services based on individual support plans

  • Train, support, and guide direct support staff

  • Ensure compliance with all 245D regulations, documentation standards, and internal policies

  • Monitor health, safety, and progress of individuals served

  • Lead team meetings and support goal planning for individuals

  • Collaborate with families, case managers, and healthcare providers

  • Maintain accurate documentation, incident reports, and progress summaries

  • Participate in on-call rotation and provide occasional direct care as needed

Qualifications

To qualify under Minnesota 245D regulations, applicants must meet one of the following education + experience pathways:

  • Bachelor's degree in a human services field + 1 year of full-time direct care experience, OR

  • Associate's degree in a human services field + 2 years of full-time direct care experience, OR

  • Diploma in a human services field + 3 years of full-time direct care experience, OR

  • 4 years of full-time direct care experience under qualified supervision AND:

    • At least 50 hours of documented training/education related to human services and disabilities

  • Supervisory or team leadership experience in social services

  • Strong working knowledge of Minnesota 245D licensing requirements

  • Valid driver's license and reliable transportation

Compensation & Benefits
  • Salary Range: $60,000–$70,000/year, depending on experience
