Position Name: Manager of Case Management Day Shifts: Days Sign ON: Case by Case Relocation: Case by Case Facility Overview: This facility is 585+ bed hospital. Our hospital offers an array of medical and health clinic services. This includes women's services offering obstetrics and fertility, an emergency room, and a trauma center. We offer a Stroke Center, a walk-in clinic, and orthopedics across three Kansas City locations. Our specialty care areas include a Chest Pain Center, Level I Trauma Center, Breast Care Center, Cardiovascular Rehabilitation Programs, and Primary Stroke Center. We are one of 10 HCA Midwest Health hospitals in Kansas City and surrounding areas Job Summary: The Manager for Case Management Services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department. The Manager for Case Management is accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team. The Manager for Case Management functions as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team. The Manager for Case Management supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines. Responsibilities:
  • Serves as a role model and functions as a clinical resource for staff.
  • Supervises and facilitates development of staff and oversees the day-to-day tasks/activities in the department.
  • Ensures staff are competent in the use of InterQual to progress the plan of care.
  • Works collaboratively and maintains active communication with physicians, nurses, and other members of the interdisciplinary care team to effect timely, appropriate patient management.
  • Monitors the quality of CM interactions with patients, families, physicians, and the multidisciplinary team by regularly shadowing staff to ensure clear understanding and adherence to department processes.
  • Monitors quality of documentation and assessments by regularly auditing the case managers' documentation.
  • Participates in clinical performance improvement activities.
  • Communicate effectively with patients, family, medical staff, care team members, students, supervisors, and outside contacts.
  • Assumes responsibility of the department in the absence of the Director.
Qualifications/Experience:
  • Currently licensed as a Registered Nurse, in Missouri, or Compact license
  • Would Prefer 1 year of Case Management RN experience (Recent acute care experience is not required. They would be open to Case Management experience that is outside an acute care setting)
  • Would consider speaking with candidates that don't have Case Management experience but have RN leadership experience like charge nurse, supervisor ETC. Acute care experience is not required.
