Care Coordinator

St. Joseph's Medical Center is seeking a full time Care Coordinator to join our Health Home Care Coordination program.

The program is designed to coordinate services and supports for adults with mental illness, chronic medical conditions, and substance abuse, and help them live independently in the community using the Health Home model of service delivery. This is an 1199 union position.

Responsibilities:

  • Care Coordinator services are mobile, provided in the community
  • The population has complex needs, including serious mental health, medical and community supports
  • Supporting engagement in medical and behavioral healthcare is the primary purpose of the program
  • Documenting Medicaid billable services in compliance with Health Home regulations is required.
  • Must be motivated, professional, and team oriented.

    • Spanish Speaking preferred but not necessary.

    Requirement:

    Bachelor's degree in related field and minimum two years' experience. Field work and home visits are required so must have valid driver's license and your own car.

    Salary: $31.7325 per hour
