Our client is OFFERING A $4,000 SIGN ON BONUS!!

The Clinician I will assess and deliver quality mental health care to youth and family in Community, Home, School, or Residential setting under appropriate supervision.

The position is working on the General Prevent team, a home visitation program geared towards strengthening families and preventing out of home placement for children.

In this position you will complete home visits with families, supporting with mental health, basic needs, safety planning, and case management.

This position provides a flexible work schedule, working as part of a supportive team, weekly Clinical supervision, and comprehensive training.

Individual must be comfortable in working in community settings such as the youth's home and school.

Responsibilities

  • Assist the Clinical team in the assessment of youth and family needs through clinical interventions to treat and achieve positive outcomes.
  • Create, implement, evaluate, and modify, as needed, individual treatment plans to meet assessed client's unique needs as a member of a multi-disciplinary team.
  • Assess and facilitate referrals to services and resources provided by the treatment team.
  • Provide individual and group therapy, as well as family therapy, based on client needs.
  • Collaborate with other informal supports, community, and mental health providers to provide needed services to clients.
  • Develop, maintain, and retain all required documentation in a compliant and timely manner including, but not limited to, clinical assessments, treatment plans, and progress notes.
  • Act as the primary contact for the treatment plan for youth being treated.
  • May provide transportation for youth as needed to appointments, meetings, or appearances.
  • Ensure policies, procedures and all applicable governmental laws/regulations/guidelines are followed.
  • Other duties may be assigned

Education & Experience

  • Master's degree required
  • Must be license eligible

Special Requirements

  • Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist, Licensed Mental Health Counselor (LMHC), or Licensed Creative Arts Therapist (LCAT)
  • If not licensed, obtain a limited permit upon hire and successfully pass licensing exam by the end of permit period.
  • Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Ability and willingness to travel
  • CPR Certification required or obtained within 60 days of hire.

  • Must be able to work a variable scheduling including evenings and weekends
  • The following physical demands are required:
  • Occasional sitting (up to 4 hours)
  • Occasional standing (up to 1 hour)
  • Occasional walking (up to 2.5 hours)
  • Occasional driving (up to 6 hours)
  • Continuous balancing (up to 8 hours)
  • Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours)
  • Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs.), 10-15 lbs. on a daily basis
  • Weekly carrying up to 55 lbs. approximately 10-100 ft., 10-15 lbs. on a daily basis
  • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
  • Occasional exposure to dust, fumes, gases or chemicals is apparent.
  • Protective clothing or equipment as required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation
  • Ability to change positions as needed

Salary/hours/work environment

  • $60,500 - $77,500 – based on experience
  • Eligible for additional $4,000 program bonus
  • Hours are mostly Monday – Friday 8:00am – 5:00pm (there may be some times when due to the families schedule you may need to work after 5pm
  • This is a community based position. You will use your personal vehicle and mileage will be reimbursed
