Job Title: Care Manager – Transition of Care
Assignment Duration: 4 months (HARD end date: December 31, 2025 – no extensions/conversions)
Work Location: Remote – Must hold Nebraska (NE) licensure
Schedule: Thursday–Monday (Flexible 2 PM–8 PM). Must work evenings and weekends. Flexible weekend hours but must complete 8 hours/day between 8 AM–8 PM. Tuesday–Wednesday OFF.
Position Purpose
The Care Manager – Transition of Care is responsible for assessing, planning, and implementing post-discharge support services to promote quality, cost-effective care for Medicaid members. This role ensures timely follow-up after acute care visits, performs medication reconciliations, and coordinates behavioral health and medical resources to improve outcomes for members.
Key Responsibilities
- Conduct telephonic outreach to members during evenings and weekends to facilitate timely post-discharge follow-up.
- Complete pre-admission and post-discharge medication reconciliations.
- Identify and coordinate complex care transition support services in collaboration with local Case Managers.
- Monitor members' compliance with care plans and connect them with behavioral health and medical resources.
- Document all assessments, communications, and follow-up actions in accordance with state and company guidelines.
- Work with care management teams to ensure members receive appropriate support after acute care visits.
- Maintain performance standards by completing required assessments within established timelines.
Required Qualifications
- Education: Master's degree in a Behavioral Health field OR Registered Nurse with extensive psychiatric experience.
- Licensure: Must hold an active Nebraska license (LMHP, LCSW, LISW, LMSW, LMFT, LMHC, LPC, or RN with psych background).
- Experience: Minimum 2 years of relevant experience.

Preferred Qualifications
- Familiarity with Nebraska behavioral health resources.
- Experience working with Medicaid populations, especially members with mental health or substance use challenges.
