Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Position Purpose
The Care Manager will develop, assess, and facilitate complex care management activities for members with serious mental and behavioral health needs. This role focuses on ensuring quality, cost-effective outcomes for Medicaid members (ages 6–64) through personalized care plans, coordination of medical and behavioral health services, and regular member engagement.
Key Responsibilities
- Conduct monthly phone calls, quarterly in-person visits, and annual assessments for assigned members.
- Develop and implement individualized care plans addressing behavioral health, medical, housing, and transportation needs.
- Coordinate care with mental health providers, physicians, facilities, and community resources.
- Monitor member adherence to treatment plans and appointment attendance.
- Ensure timely and accurate documentation of all member interactions within 24 hours (Friday exceptions noted).
- Meet state-driven metrics and deadlines for outreach and documentation.
- Travel to member homes, residential programs, and mental health facilities as required.
- Collaborate with peers, supervisors, and assigned “buddy” case managers for co-visits and support.
- Participate in ongoing training, adapt to policy and procedure updates, and maintain compliance with company and state guidelines.
Required Qualifications
- Education: Bachelor's degree in a health-related field.
- Licensure: Active RN license required; LMSW may be considered.
- Experience:
- Background in behavioral health strongly preferred.
- Experience in case management and member-facing work.
- Skills:
- Strong adaptability to frequent changes in policies and procedures.
- Ability to work independently while meeting strict deadlines and performance metrics.
