S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Patient Care Coordinator's primary function is to schedule patient services and coordinate with staff. Position Specific Responsibilities. Schedules and appropriately documents patient schedules in a timely manner. Participates in coordinating care with management and patient interdisciplinary team. Provides effective communication to patients, staff members, other health care professionals, and referral sources. Responds to agency and patient needs in a professional and creative manner.
