Responsible for providing individual therapy and/or counseling services to Health Center patients. This position also provides input and support for staff involved in the care of the patients. Primary Responsibilities:Perform psycho/social assessment. Develop a therapeutic counseling regimen to meet the needs of the individual based upon assessment and input from caregivers/family. Coordinate treatment plan with the psychiatrist, referring individuals as necessary for psychiatric consult and prescription. Provide ongoing counseling as indicated and follow up if a psychiatric consultation is made. Maintain patient records per agency policies and procedures. Provide ongoing supervision of MS - Ws, as necessary and appropriate. Perform other duties as requested by the supervisor. Requirements Masters Degree in Social Work required.
