Position Overview:A mission-driven, community-focused healthcare organization is seeking a compassionate and skilled Family Medicine Physician (MD or DO) to join a dynamic team in South Bend, Indiana. Key Responsibilities:Provide comprehensive outpatient primary care to a diverse patient population
Collaborate with a multidisciplinary team including physicians, nurse practitioners, dentists, and behavioral health specialists
Maintain accurate documentation using eClinicalWorks (eCW) EMR
Participate in quality improvement initiatives and team-based care
Support and embody the mission of providing compassionate, affordable care to all
Monday to Friday daytime hours (no evenings)
Four-day workweek optional
No weekends required
Major holidays off
Dedicated administrative time included in weekly schedule
100% outpatient care - no hospital or inpatient responsibilities
18-20 patients per day for quality-focused care
Integrated behavioral health services for holistic treatment

M.D. or D.O. from an accredited medical school
Completion of an accredited residency in Family Medicine
Board-Certified or Board-Eligible
Eligible for medical licensure in the state of Indiana

Prior experience or interest in working within FQHC, CHC, or PCMH settings is preferred
