POSITION SUMMARY: As a Home Health Admissions Nurse, you will deliver exceptional intermittent nursing services to patients in their homes as per the established plan of care, which is developed in collaboration with the patient's physician and aligned with agency policies and procedures. Your responsibilities will also include admitting clients to services, establishing care plans, and referring to other disciplines and agencies as needed.

EDUCATION/CERTIFICATION: Required to be a graduate of an NLN-approved school of nursing with either an associate or baccalaureate degree. Possession of a current Registered Nurse (RN) license in the State of Connecticut is mandatory.

EXPERIENCE: Minimum of one (1) year of recent medical/surgical nursing experience is required; additional experience in community health nursing is preferred.

COMPETENCIES: Must possess:

  • Understanding of agency policies, regulations, and procedures.
  • Proficiency in medical equipment used for patient care.

  • Effective documentation and use of electronic medical records proficiency.
  • Ability to foster relationships with patients, families, and healthcare professionals.
  • Capability to maintain quality control and react calmly in emergencies.

  • Reliable transportation and proof of adequate automobile insurance as per State of CT regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES: This is not an exhaustive list of responsibilities. It reflects the primary duties and may change over time:

  • Admit/readmit patients and provide nursing services in compliance with agency policies and regulations, incorporating:
  • A thorough physical and psychosocial assessment.
  • Development and implementation of care plans and therapeutic goals with the patient/family.
  • 24-hour patient safety evaluation.
  • Effective clinical judgment in coordinating patient care across services.
  • Ongoing evaluation of patient condition and progress towards goals.
  • Utilization of agency and community resources and consultants.
  • Patient/family education on care responsibilities and wellness practices.
  • Planning for discharge from services.
  • Maintain quality care metrics aligned with agency goals, including:
  • Accurate OASIS documentation related to care plans.
  • Documentation completion within 36 hours of visit.
  • Coordination of care between physicians and disciplines.
  • Effectively manage work assignments by:
  • Visiting all new clients within 24 hours of referral.
  • Reporting any at-risk situations to the Supervisor of Clinical Services.
  • Remaining adaptable in daily planning to accommodate emergencies.
  • Meeting established productivity metrics.
  • Supervise and orient Home Health Aide according to established plans and regulations.
  • Continuously develop professional skills and credentials through ongoing education.
  • All other duties as required in support of agency programs, including participation in weekend/holiday rotations. This role may require back-up for on-call services when needed.
