Position: Community Health Worker (CHW)Location: Evanston Hospital. Full Time/ Part Time: Full Time. Hours: Monday-Friday, 8:00 AM-4:30 PM. In addition, there may be occasional evening or weekend hours for community events. Travel: Frequent local travel will be required. Community Health Workers support patients in the ambulatory care setting and community as needed. You must either maintain a valid State of Illinois Drivers License with proof of active insurance or have access to other means of transportation. What you will do:Assess and address patients needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patients health outcomes (Social Determinants of Health). In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.
