15319 BRFTE: 1.0 Shift: Weekdays 8 a-4:30 p. Will travel to all clinics as needed. Compensation: Starting at $21.20, commensurate with experience. Hours Per Week: 40 Weekends: Benefit Eligible. The Community Resource Specialist role is essential to provide UnityPoint Health Meriter medically homed patients with comprehensive care and work to address social determinants of health. This position collaborates with other clinical staff to assess and triage patient social service and community resource needs and coordinate services and referrals as appropriate.
