HouseCalls Home Care is a leading Licensed Home Care Services Agency (LHCSA) committed to providing compassionate, high-quality care to elderly and disabled individuals across New York. We're seeking a Bilingual Patient Care Coordinator (Spanish-speaking) to join our team in our Brooklyn office.

This role plays a vital part in helping patients receive personalized services in the comfort and dignity of their homes.

Key Responsibilities

  • Serve as the primary point of contact for patients and families
  • Develop and coordinate customized home care plans
  • Schedule services, follow-ups, and in-home assessments
  • Educate patients and caregivers on conditions, treatments, and home care routines
  • Monitor and document patient progress and interventions
  • Collaborate with healthcare providers, aides, and specialists
  • Ensure compliance with care protocols, agency policies, and health regulations
  • Provide respectful, responsive, and empathetic support at all times

Qualifications

  • Minimum 1 year of experience in care coordination, case management, or clinical support — specifically within home care or a closely related healthcare setting
  • Fluency in Spanish is required
  • Excellent verbal and written communication skills
  • Proficiency in Microsoft Office and electronic health record systems
  • Ability to multitask and thrive in a fast-paced environment
  • Strong organizational skills, empathy, and attention to detail
