Responsibilities:

• Deliver compassionate, evidence-based care to patients with chronic or critical conditions
• Collaborate with interdisciplinary teams to develop and implement care plans
• Monitor patient progress and adjust treatments as needed
• Maintain accurate documentation and ensure compliance with hospital protocols
