Assist in identifying and caring for individuals with diabetes mellitus. This will include type 1 and type 2 diabetes as well as patients on insulin pumps. This position will allow growth into other endocrinology disorders such as thyroid disease, and bone health. Identify at-risk patients, provide evidence-based nutrition and disease specific educational interventions, coordinate care with team members, and prevent avoidable utilization and adverse outcomes. Interprets continuous glucose monitor and pump readings. Provide teaching on new insulin administration, meter teaching, insulin pump malfunctions/review Provides diabetes counseling and education to patients and families as well as plan and put together dietary meal plans with patients based on their medical conditions, restrictions and needs.
