Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 09/02/2025 Duration: 13 weeks 37 hours per week Shift: 12 hours Employment Type: Travel Travel RN – Case Management Location: White Plains, NY Start Date: 9/2/2025 Duration: 13 weeks Schedule: 5x8s, 9:00 AM – 5:00 PM Guaranteed Hours: 37.5 per week Care Setting: Acute Care Charting System: Epic Requirements: Current NYS RN license Minimum 2 years of nursing experience (ED, critical care, or med/surg strongly preferred) BLS (AHA) required PRI certification (must be obtained within 3 months of employment) Screen certification strongly preferred Previous case management experience preferred Care Management certification (CMC, CCM, etc.) preferred Experience with Milliman/InterQual criteria Strong knowledge of healthcare financial, regulatory, and payer issues Knowledge of state, local, and federal programs strongly preferred Proficiency in Epic and strong computer skills Unit/Role Notes: The RN Care Manager is responsible for comprehensive patient assessments, utilization review, and discharge planning . This includes screening patients using established tools for high-risk indicators, coordinating care across disciplines, and ensuring safe and appropriate discharge planning. Key Responsibilities: Complete initial and ongoing patient assessments Perform multidisciplinary care coordination at both intra- and inter-hospital levels Manage all transitional planning activities including patient/caregiver education, aftercare arrangements, and payer communication Collaborate with health team members and ensure effective communication of patient needs Conduct utilization review activities per department policy (payer reviews, admission clinical reviews, variance management) Support department goals of LOS reduction, readmission prevention, denial reduction, and patient satisfaction improvement Participate in performance improvement initiatives Preferred Background: 3 years of ED, critical care, or med/surg experience Prior case management experience in an acute care hospital Strong communication and interpersonal skills Why Travel with HumanEdge Health?
