: Job Details :

Coordinates and plans all transportation trips for participants for Senior TLC daily attendance, specialty appointments, discharge from SNF (Skilled Nursing Facility) & hospital, daily dialysis trips including weekend and holidays. Works independently and makes scheduling decisions for the organization based on resource allocation and participant need. Determines the safest and most efficient pick-up and delivery routes. Receives feedback from Drivers to integrate into transportation schedules. Daily receives information from IDT meetings and care planning as scheduled. Effectively communicates with participants, families, staff and other agencies and departments. Provides training and support to team of Drivers in collaboration with the Transportation Manager to assure competence in driving all variety of vehicles that are included in the organization's transportation fleet.
