Assists with assessment, intervention, and follow-up of referred patients. Provides case management including continuum care planning and advocacy. Serves as department community resource development specialist and as a liaison between select patients of all ages and community service systems. Performs other job duties as required. Qualifications
EDUCATION
Bachelor of Social Work degree from a program accredited by the Council on Social Work Education or equivalent bachelor's degree with experience in working in a related helping profession or with community agencies.
EXPERIENCE
May be a new graduate. Preference is given if experienced in health or mental health social work or related helping profession.
SPECIAL SKILLS
Competency in caring for patients treated at the Hospital regardless of age, disability, sexual preference, gender, ethnicity, or diagnosis. Computer literacy.
