The ideal candidate will possess experience in outpatient behavioral care, with a focus on patient-centered assessment, education, and comprehensive care. Key Responsibilities: Assessment and Patient Care: - Conduct initial assessments and document patient needs for immediate nursing action. - Systematically assess nursing requirements and document findings to monitor changes in the clinical picture. - Develop, implement, and evaluate nursing care plans based on comprehensive assessments. - Address and document the educational, psychosocial, and discharge planning needs of patients. - Initiate referrals to other departments or agencies as necessary for holistic patient care. Direct Patient Care: - Perform blood draws for lab testing. - Provide comprehensive patient care and follow through with family education. - Consult with nursing leadership regarding significant changes in patient conditions. - Communicate relevant patient information during shift handovers. - Utilize established patient care standards in clinical practice and document patient responses effectively. Indirect Patient Care Activities: - Participate in interdisciplinary rounds, presenting patient findings and specifics related to disease management. - Maintain principles of infection control and adhere to safety standards. - Embrace and uphold the organization's Mission, Vision, and Values. - Provide exemplary customer service, characterized by respect, professionalism, and support for patient experience. Qualifications: - Minimum of 1 year experience in behavioral health care, preferably in an outpatient setting. - Current certifications in BLS and CPI or PMCS. - Strong clinical assessment and communication skills. - Proven ability to work collaboratively in a healthcare team environment.
