Healthcare Consultant III Location: Remote - Must Reside in or Near Winnebago County, IL Schedule: Monday - Friday | 8:00 AM - 5:00 PM CT | Full-Time Overview: We are seeking a dedicated and experienced Healthcare Consultant III to join our care coordination team. This is a remote position with regular travel required to visit members in their homes throughout Winnebago County, Illinois. The ideal candidate will bring a strong background in behavioral health or human services, excellent organizational skills, and a passion for empowering individuals through compassionate and resource-driven care. Key Responsibilities: Conduct comprehensive assessments of referred members to determine needs and service eligibility.
