Job Description :

The Registered Dietitian is responsible for providing expert nutrition and dietary guidance to individuals or groups to promote optimal health. This includes assessing nutritional needs, developing personalized meal plans, educating patients and collaborating with the entire Health and Care Support Team. The RD ensures that all recommendations align with current scientific guidelines and regulatory standards.

Essential Job Functions

  • Conduct one-on-one nutritional assessments via telehealth for individuals based on medical history, lifestyle, and dietary habits.
  • Develop and implement personalized care plans and intervention for chronic disease management and prevention.
  • Monitor and evaluate the effectiveness of dietary plans, adjusting as needed.
  • Complete session documentation, including clinical notes, patient communication, and care team follow-up messages in a timely manner.
  • Apply motivational interviewing concepts to help members problem solve difficult life situations such as social gatherings, eating out, holiday meals and travel.
  • Educate patients on the use and benefit of Medically Tailored Meals.

Qualifications:

  • Minimum of a bachelor's degree at a US regionally accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND) of the Academy of Nutrition and Dietetics.
  • Completed an ACEND-accredited supervised practice program at a health-care facility, community agency, or a foodservice corporation or combined with undergraduate or graduate studies.
  • Passed a national examination administered by the Commission on Dietetic Registration (CDR).
  • State licensure/certification in PA and NY
  • Minimum 2 years of experience in clinical nutrition, community health, or chronic disease management
  • Proven experience using Motivational Interviewing techniques to support behavior change
  • Demonstrated ability to manage and monitor patients with chronic conditions such as: Diabetes, Hypertension, Heart disease , Obesity , Renal disease
  • Experience working in or with Remote Patient Monitoring (RPM) platforms and workflows
  • Experience with Electronic Health Records (EHR) and telehealth platforms
  • Ability to multitask with members and operationally with managers and support staff
  • Bilingual in English and Spanish preferred
  • Active credentialing with commercial insurance plans, Medicare and Medicaid preferred
  • Certified Diabetes Educator preferred

Total Rewards Package

Base Salary: $65,000 annually

Performance Bonus: Earn up to an additional 15% each year for meeting performance goals
