Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patient/family, and referring agency. Assume responsibility for coordination of care. Assess home health and/or hospice patients in stable and emergent situations. Implement, develop, document, and evaluate the Plan of Care (POC). Obtain data on physical, psychological, social, spiritual, and cultural factors influencing health status. Maintain up-to-date patient records reflecting all visits, contacts, problems, plans, actions, goals, and changes. Provide care utilizing infection control measures. Educate patients and/or their families regarding care, diet, medications, home safety, etc. Supervise and provide clinical directions to Home Health Aides (HHA), State Tested Nursing Assistants (STNA), Licensed Vocational Nurses (LVN).
