Position: Registered Nurse - Discharge Coordinator

Full-Time

What You'll Do:

As a Registered Nurse, you will play a vital role in ensuring the health, comfort, and overall well-being of our residents. Your primary responsibilities will include assessing patient conditions, creating and implementing personalized care plans, administering medications, and collaborating with healthcare professionals to provide holistic care.

Key Responsibilities:

  • Conduct thorough patient assessments and develop individualized care plans
  • Administer medications, treatments, and IV therapies as prescribed
  • Monitor and document patient vital signs and medical information
  • Provide wound care and other necessary nursing interventions
  • Collaborate with the interdisciplinary team to guarantee comprehensive care delivery
  • Supervise and mentor Licensed Practical Nurses (LPNs) and Certified Nurse Assistants (CNAs) in providing quality care
