The Assessment Referral Coordinator supports patient care by coordinating referrals, authorizations, and follow-up appointments in collaboration with physicians and other care team members. This role ensures timely scheduling, accurate documentation, and clear communication with patients to facilitate continued access to services and high-quality care. Essential Functions Coordinates patient referrals, authorizations, and appointments for specialty care, diagnostics, and follow-up services in accordance with provider orders. Communicates with patients and families to explain next steps, provide instructions, and ensure understanding of referral and appointment requirements. Obtains prior authorizations and referral approvals as needed, and accurately documents information in the electronic health record.
