Join our compassionate team in Philmont, New York, where you'll help residents regain mobility, strength, and independence in a supportive Skilled Nursing Facility (SNF) environment.Key Responsibilities:
- Perform comprehensive evaluations to assess residents' mobility, strength, and therapy needs.
- Develop and implement personalized treatment plans to improve residents' functional abilities and quality of life.
- Provide therapeutic interventions, including exercises and modalities, to help residents achieve their goals.
- Monitor progress, adjust treatment plans as needed, and provide education to residents and their families.
- Collaborate with an interdisciplinary team to deliver exceptional patient-centered care.
- Document assessments, treatments, and progress accurately and in compliance with facility and regulatory guidelines.
Qualifications:
- Current Physical Therapist (PT) license in New York.
- Graduate of an accredited Physical Therapy program.
- Experience in a Skilled Nursing Facility (preferred but not required).
- Strong communication and interpersonal skills.
