Albright Life Centers are expanding our Primary Care Providers. Come join our team of caring team of primary care providers led by a knowledgeable, compassionate Medical Director.

This position rotates between Albright LIFE's Cumberland and Franklin Center locations.

LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their healthcare needs in the community instead of going to a nursing home or other care facility. At an Albright LIFE location, you'll work with a team of healthcare professionals providing exceptional, coordinated services each day that help participants stay healthy and safely live independently at home.

Responsibilities include:

  • Provides comprehensive primary care services to Albright LIFE participants.
  • Ensures ongoing coverage and continuity; coordinates with other LIFE providers for on call coverage and participates in that coverage as needed.
  • Acts as medical liaison between LIFE program and others in the medical community (i.e. primary physician groups (attending), specialists and hospital staff); promotes discussions regarding medications, treatments, discharge and alternative regimens.
  • Ensures compliance with LIFE provider regulations as they pertain to medical/clinical components.
  • Compassion, patience, and genuine desire to make a positive impact on the lives of seniors.

Qualifications

Requirements include meeting all conditions of participation in CMS:

  • Doctor of Medicine or Doctor of Osteopathic Medicine
  • Board certified in Geriatric Medicine or Family Medicine
  • Licensed to practice in the Commonwealth of PA.
  • Medical care knowledge and skills essential for caring for frail elderly, including a minimum of one year experience working with frail or elderly populations.
  • Demonstrates leadership abilities and the ability to coordinate and facilitate teamwork and interdisciplinary groups. Demonstrates positive interpersonal relations in dealing with others, so that productivity and positive interpersonal relations are maximized.
  • Consistently demonstrates and incorporates principles of safety and infection control into daily activities
