Job Title: Case Management Assessment Specialist – Remote (RN, California Licensed)

Employment Type: Contract | Full-Time

Location: Fully Remote – Anywhere in the U.S.

Schedule: 9:00 AM – 5:30 PM PST

Pay Rate: $30 – $49/hour

Job Summary

Wheeler Staffing Partners is seeking a Case Management Assessment Specialist (RN) to join a remote care management team. This role is responsible for assessing select member populations, identifying health and psychosocial issues, and developing individualized care plans to maximize the health and wellbeing of members.

The ideal candidate is a California-licensed RN with case management experience and prior experience working remotely.

Key Responsibilities

  • Identify physical and mental health issues affecting members and collaborate with members and providers to determine appropriate care.
  • Conduct Comprehensive Functional Needs Assessments (CFNA) and develop Integrated Plans of Care (ICP) for short- and long-term needs.
  • Communicate with members, families, physicians, and providers to gather medical history, assess current status, and determine care options.
  • Maintain and document CFNA and ICP in the medical management platform accurately and efficiently.
  • Schedule member appointments, follow-up services, and consultations; provide appointment reminders as needed.
  • Ensure strict confidentiality and adhere to HIPAA regulations.
  • Monitor case volume, task deadlines, and follow-up activities in the medical management platform.
  • Perform member outreach to schedule assessments and follow-up care.

Required Qualifications

  • Active Registered Nurse (RN) license in California (current and unrestricted).
  • 2+ years of clinical acute care experience (home health, physician's office, or public health preferred).
  • Case Management or managed care experience.
  • Strong critical thinking, problem-solving, and organizational skills.
  • Excellent verbal and written communication skills.
  • Comfortable working independently and collaboratively in a remote environment.
  • Proficiency with medical management platforms; experience with MS Word, Excel, and Outlook is a plus.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • 1–3 years of experience in disease management, case management, or patient education.
  • Certified Case Manager (CCM) credential.

Knowledge, Skills, and Abilities

  • Knowledge of disease-specific states, clinical protocols, and community resources.
  • Ability to advocate and communicate assertively with members, providers, and team members.
  • Ability to manage multiple projects and cases simultaneously under strict timeframes.
  • Strong commitment to patient care and helping others.
