Community Health Worker/Case Manager

We are seeking a compassionate, motivated, and culturally responsive bilingual (Spanish/English) Community Health Worker to support pregnant and parenting youth in achieving long-term health, educational, and economic success.

PROGRAM DESCRIPTION: The Young Parent Program at Child Care of the Berkshires partners with pregnant and parenting youth ages 14-25 to increase life opportunities and enhance family stability. The program utilizes a strengths-based, two-generation case management model that focuses on participants' individual needs. Staff support parents in reaching their educational and career goals, increasing housing stability, access to health care and child care, and building positive parenting skills.

POSITION SUMMARY: The Community Health Worker/Case Manager will serve as a trusted advocate and liaison, connecting young parents with vital resources to promote family stability and overall well-being. This role emphasizes individualized support, education and employment advancement, emotional wellness, and community resource navigation. This position is located in Pittsfield, but will include travel to surrounding communities and occasional trips to our North Adams office.

QUALIFICATIONS:

  • Fluent in both Spanish and English, with the ability to conduct visits with families in both languages while documenting work and receiving supervision in English.
  • High school diploma or equivalent required; associate's or bachelor's degree in social work, public health, human services, or a related field preferred.
  • Lived experience as a young parent and/or prior experience working with pregnant or parenting youth is highly valued.
  • Strong understanding of issues affecting young parents, including access to education, healthcare, and housing.
  • Familiarity with trauma-informed and culturally responsive practices.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively in a fast-paced, community-based environment.
  • Must be willing to complete an EEC background check (MA criminal offender record check(CORI), SORI, DCF background check, National Sex Offender Registry (NSOR)) fingerprinting, and a motor vehicle check.

WORK SCHEDULE AND YEAR: This is a 20-40 hours per week, year round non-exempt position.

SALARY AND BENEFITS: $23-$25/hr, commensurate with experience. Benefits include vision and dental insurance, health insurance eligibility starts at 30 hours per week, sick, personal, and vacation time as well as paid holidays, paid closure days, and retirement plan.
