Case Management Services

Provides comprehensive, structured, case management services for emergency shelter residents. Learns and adapts to the uniqueness of a resident's history to determine the most effective relationship approach. Develops comprehensive program plan/goals and evaluates residents' progress through one-on-one sessions and diagnostic tools. Advocates for community resources and services that will enable residents to resolve or ameliorate the underlying causes of their homelessness.

Key responsibilities:

  • Conducts resident intake and an initial needs assessment at the time of entry to the shelter. Provides linens, toiletries, and hygiene products; replenished as needed throughout residents' stay.
  • Meets individually with residents to discuss and evaluate their progress, feelings, impressions, changes, and personal growth/development; prepares accurate and up-to-date records documenting the same.
  • Prepares and maintains case records and logs; ensures the accuracy and completeness of the same; enters pertinent information into the Homeward Community Information System (HCIS).
  • Refers residents to additional counseling resources if needed. Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.

What we are looking for in you:

  • Associate degree in social work, human services, or related field from an accredited college or university; AND
  • One year experience providing direct case management social services including accessing residents' needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources; OR
  • Any equivalent combination of training and experience providing the required knowledge, skills, and abilities.

Physical Requirements and Working Conditions:

Work is performed in a shelter facility where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like. Ability to meet attendance requirements. Ability to read, write, and communicate the English language. Ability to communicate clearly and effectively both orally and in writing. Ability to work on multiple cases effectively and efficiently without becoming frustrated or disorganized. Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.
