The Case Manager II (CMII) coordinates and facilitates patient care across a multidisciplinary care continuum within a clinical service unit structure. The CMII supports, while providing psychosocial assessment and supportive counseling to patients and families to facilitate clinical improvement and discharge. Services provided include: psychosocial assessment; advance directive counseling; chemical dependency screening and referral; processing of guardianship issues; identification and crisis intervention for victims of abuse/neglect; provides knowledge and referrals to available community resources. All activities are focused on assisting the patient, family, and team to address psychosocial issues to ensure referral and mobilization of available community resources.
QualificationsBachelor's degree in the field of human services, psychology, and sociology, or social work or related healthcare field required
Illinois Licensed Social Worker required obtained within two years of hire; Certification in Accredited Case Manager (ACM) and/or Chronic Care Management (CCM) preferred
Five to seven years of inpatient case management experience
Good communication and interpersonal relationship skills
Able to organize work with minimal supervision
Able to focus attention to minute details
Access, Word and Excel computer skills required

Serves as a super user for EHR and other case management tools.
Demonstrates clinical competency by completing BLS CPR and other training modules for colleagues assigned by leadership.
Performs other duties as assigned.
