Case Manager
: Job Details :

Amergis is seeking a qualified Case Manager to work at a leading provider of home and community-based specialized health care. This would be a full-time, day-shift role and would be an immediate start. See below for full details:

Location: Springfield, IL 62704

Schedule: 8:00am-5:00pm, Monday-Friday

Duration: 13 week assignment with an option to extend or go internal

Pay Rate: $30/hr + Mileage Reimbursement

Caseload: 4-6 home visits per shift, each home has 3 individuals, homes are located within Springfield, IL and Chatham, IL

Experience Required:

• QIDP certification REQUIRED
• 1+ years of experience in Case Management with individuals with developmental disabilities
• Bachelor's degree in human services, psychology, or equivalent in education and exp required
• Valid driver's license, registration and insurance
• Excellent documentation and Communication skills preferred
