22nd Century Healthcare is seeking a Social Work Correctional Social Worker for a local contract job in Buena Vista, Colorado.

Responsibilities:

• Provide comprehensive mental health care to a specialized inmate population with mental illness, intellectual and developmental disabilities, and individuals at high risk for suicide and self-injury.
• Manage a caseload primarily consisting of inmates with moderate to high mental health treatment needs.
• Develop individualized mental health treatment plans.
• Provide individual and group therapy sessions.
• Conduct routine mental health assessments and interviews to monitor patient progress.
• Update diagnoses and psychiatric care levels as needed.
• Refer inmates for complex psychological assessments as necessary.
• Provide client history and treatment information to psychiatry.
• Coordinate emergency and non-emergency transfers for inmates experiencing acute mental health crises.
• Provide clinical guidance to non-clinical staff involved in inmate mental health care.
• Ensure continuity of care by facilitating appropriate referrals and sharing necessary documentation during inmate transitions.

• Valid, active license from Colorado or a participating Social Work Licensure Compact state as one of the following:o Licensed Professional Counselor (LPC)o Licensed Clinical Social Worker (LCSW)o Licensed Marriage and Family Therapist (LMFT)
