Job Overview

Deliver compassionate end-of-life care in patients' homes, managing hospice and home health needs. Rotate on-call coverage for after-hours emergencies.

Key Responsibilities

• Provide pain management, symptom control, and emotional support to hospice patients.
• Collaborate with interdisciplinary teams for personalized care plans.
• On-Call Duties:
  • Weeknights (20:00–08:00) and weekends (Friday 16:30–Monday 08:00).
  • Cover both Home Health and Hospice during on-call shifts.
• Document care in compliance with facility standards.

Requirements

Licensure & Certifications

• Active RN license (MS state or compact).
• BLS certification (AHA preferred).
• Valid driver's license and auto insurance (required for home visits).
